What a Drifting Eye Tells You About Head Position

You have probably seen this case. Primitive reflexes integrated. Postural reflexes developed. Vestibular testing looks reasonably clean. And convergence still will not hold on one side.

Before you go deeper into ocular work, read the eye as a head position finding.

What a drifting eye tells you

If the left eye sits out, that tells you the head has been chronically rotated to the right. Which tells you the left vestibular system is the weak side. The eye is the output. The canal is the driver.

Surgery changes the picture, not the driver

When a child has had strabismus surgery, they went in and shortened a muscle. If the right eye was sitting out, the medial rectus gets shortened to bring it toward midline so the child can get single vision. That is what the procedure is designed to do, and it does it.

What it does not do is resolve the vestibular asymmetry underneath. That weakness is still there, just harder to see, because the eye now sits in a position the brain is not expecting. In my experience these kids are significantly harder to rehab for exactly that reason.

So ask which eye, and ask which muscle. Sometimes the right eye was corrected and the left began to deviate afterward. Then you are looking at the right horizontal canal, not the left.

Horizontal or vertical tells you the canal

If the misalignment is purely horizontal, work the horizontal canal. If an anterior canal is weak, the eye does not elevate efficiently, so you will see a vertical component alongside the horizontal one. No vertical component, no anterior canal.

What actually moves the needle

Test it before you build a plan around it. Five to ten quick head thrusts toward the involved side, slow return the other way. Then recheck convergence. If convergence comes easier, you have your target, and you can load it with slow rolls or slow spins to that side, ten to fifteen reps at a time.

Final thoughts

This is also why a convergence gain can show up on day two of an intensive and be gone by day four. The eye was never the problem. Drive the canal, and the eye tends to follow.
